1.  Elvina (Alvina) Roloff was admitted to Southwest Washington Medical Center hospital on February 23, 2001 with a breathing problem. {11}

2.  M. Cheatham said that she fears Elvina (Alvina) Roloff's son will prolong her life using better medical care. {50}

3.  M. Cheatham blocked medical help from Elvina Roloff's son. {12, 15, 35, 42}

4.  M. Cheatham blocked medical help from a medical expert, Dr. Y. Chen, MD, PhD. {34}

5.  M. Cheatham said to hospital staff that Elvina Roloff was DNR. Elvina Roloff did NOT say that she was DNR to hospital staff, her son or to grandchildren. {30}

6.  Condition for DNR to become effective was NOT met. {8}

17.  Hospice SW nurse home visit was blocked by M. Cheatham at 10:30 AM, Feb 7, 2001. {54}

18.  Private Nurse started home care of Elvina (Alvina) Roloff on Feb 7, 2001. {54, 60}
