Hospice Southwest, Southwest Washington Medical Center. Nursing On-Call Documentation. Patient Name: Elvina Roloff. Telephone call from: Marie Cheatham. Comments: M. Cheatham reported patient having increased pain issues. Also blood in catheter tubing (clots). Plan: Ativan 0.5 mg po/sl Q 2 hrs prn (by mouth / sublingual every 2 hours as needed). Roxanol (morphine) 1.0 cc po/sl Q 1-2 hours (by mouth / sublingual every 1 to 2 hours). Give those meds routinely to keep Sx (symptoms) under control. PCN (Primary Care Nurse) to addess blood in catheter on HV (home visit) today. Repeat dosage on these meds within 30 minutes if no sign of good Sx control initially. Note: The nurse arrived at Elvina Roloff's home at 10:50 AM. Marie Cheatham refused to let the nurse see Elvina Roloff. Note: No other person independently verified that Elvina Roloff was having increased pain.
